On the fourth level when you kill the skeleton guys they fall off the platform into the black mist and drop skulls there which results in the inability to get 100%. Also the hood leading to the skull guys is kind of hard to get on to.

I can't get to the 3rd level on the map. Can you tell me how? Also what are the skulls you get from chests and enemys for.
