Using keyframes is probably easiest, but if you want something that looks more like actual smoke, that kind of "billows" upwards or something, you can achieve a lot with just the painting tweak settings. It sort of requires that you paint in the right direction, and it can take a lot of trial and error to figure out what each setting does. There are lots of possibilities though (not just for smoke!), and it might be cheaper as there's no logic involved.
